Mental Health Support Services

Our goals for Mental Health Support Services is to prevent institutionalization and repeated in-patient stays of clients (ages 18 and up) through the development and implementation of flexible, client-oriented programs designed to assist the client in living an active and productive life within their community.

Some of the areas we can assist with are

  • Accessing transportation
  • Accessing medical care
  • Finding housing
  • Developing skills to maintain housing
  • Accessing community resources to ensure healthy living (eg. food pantry)
  • Handling financial responsibilities
  • Advocating regarding mental health treatment, including medication education
  • Finding legal resources
  • Accessing benefits like Medical Assistance, SSI, SSDI, etc.
  • Reducing hospitalization
  • Improving family relationships
  • Improving support network
  • Maintaining employment

These services are provided in the home and the community.
