Mental Health Support Services
Our goals for Mental Health Support Services is to prevent institutionalization and repeated in-patient stays of clients (ages 18 and up) through the development and implementation of flexible, client-oriented programs designed to assist the client in living an active and productive life within their community.
Some of the areas we can assist with are
- Accessing transportation
- Accessing medical care
- Finding housing
- Developing skills to maintain housing
- Accessing community resources to ensure healthy living (eg. food pantry)
- Handling financial responsibilities
- Advocating regarding mental health treatment, including medication education
- Finding legal resources
- Accessing benefits like Medical Assistance, SSI, SSDI, etc.
- Reducing hospitalization
- Improving family relationships
- Improving support network
- Maintaining employment
These services are provided in the home and the community.
